[ARCHIVED] use this form if you need help getting covid vaccine

We will help everyone who wants the vaccine get one! The Covid Angels are doing an amazing job. To help expedite an efficient process please complete the form

The Greenburgh Covid Angels have been successful helping hundreds (probably 1,000+) of senior citizens receive their vaccinations.  They have initiated  over 2,000 phone calls to seniors. They have responded to inquiries. Almost 300 volunteers are ready, willing and able to help make the process of getting a vaccine less stressful for those who are not great with computers or who can't figure out how to get the vaccine.

To make the process more efficient we have a form that we would appreciate your completing. The Covid Angels will process the application and help you get the vaccine.
It's getting easier to get vaccinations. We worked with Rite Aid this past weekend - they administered 750 vaccinations at the Theodore Young Community Center in Greenburgh. Local pharmacies, the Greenburgh Health Center are also working with us. And more vaccination sites are opening up.  Don't get stressed out if you haven't received the vaccine yet. Our committee will be there for you until you get the vaccine.
